Tests: add IPv4-mapped IPv6 normalization test for PP v2.

Test that a stream proxy with PROXY protocol v2 enabled on a
dual-stack [::]:port listener (ipv6only=off) normalizes an
IPv4-mapped client address (::ffff:x.x.x.x) to an AF_INET
address block rather than emitting an AF_INET6 header.
